The contact form asks for your name, email, phone number, how and when to reach you, and a short message. We use it to reply to you about a visit and for nothing else. Please don't put medical details in the form.

We collect your health history on our private patient portal, not by email or through this website. We keep your records private and share them only as the law allows.
