Use the form or call (815) 555-0101. Norberto finds a time and answers questions about fees.
Fill in your health history
On our private patient portal, not by email or in the contact form. We'll send the link when you book.
Bring a few things
Your photo ID, your insurance card if you have one, and a list of the medicines you take.
Come in
You talk, Dr. Bolden looks, and you leave with what she found and what it would cost in writing.
At the chair
Slow is fine
Tell us at the start if you're nervous or if a past visit went badly. We'll go in the order you choose, and we stop whenever you ask.
The room is quiet, the lights are warm and you can bring someone with you.
